The customer received an ICF grant funded by Welsh Government to support alternative approaches to services to older people. As a result, they delivered 28 projects all funded at different levels from £500 up to £300,000 within the Cwm Taf region.

The main aims of these projects were to:

  • maintain the well-being of older people in preparation of the Social Services and Well-being (Wales)Act
  • support the speedy discharge of older people from hospital
  • support older people to live as independently as possible in their own homes.

We were commissioned to support the implementation and the evaluation of these projects.

The project took nine months to complete and drew on our associates’ expertise, capacity and commitment to deliver long-term projects such as this.

Our team helped those working in Cwm Taf to demonstrate a range of outcomes that were achieved through the implementation of the ICF programme.

Through the development of an effective evaluation framework, the customers were able to decide what projects were most effective and could be given future funding. A Welsh Government event was held at the end of the project to showcase the Cwm Taf projects and all regions in Wales were invited.
